There are three ways to process a win loss document request: fax, mail, or in-person at the Rose Rewards Club.

Download and complete Win / Loss W-2G Winnings form and bring to the Rose Rewards Club.

To avoid processing delays make sure the form is completed.
Request forms are processed within 10-14 business days.
W-2G documents are mailed to the address listed on the request form.
